How to obtain your Form 1095.
Form 1095 documents the health insurance you and your family had through WhiteWater Express for the year. To request yours, send an email to the HR department at HR@whitewatercw.com.
State that you are requesting your Form 1095 and provide an email address or US mailing address where you would like it sent. Please say clearly which option you prefer, email or US mail, because we cannot use email unless you expressly choose it.
